Lynbrook Restorative Therapy and Nursing has a Per diem opportunity for a Registered Nurse Supervisor to join their team.

PRIMARY FUNCTIONS (Including but not limited to):

RN's are responsible for the overall operation and optimal quality of care for the residents on the assigned unit.

Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facility's administration and Nursing Policies and Procedures.

DUTIES AND RESPONSIBILITIES:

Contacts the resident's physician and family to notify them of a change in the resident's condition or accident/incident; also notifies the supervisor covering.
Assumes all responsibilities of the RN Supervisor in his/her absence or as directed by the DNS/ADNS.
Assumes independent responsibility for completing the required routine documentation and auditing as required.
Must have strong communication skills and the ability to lead and motivate the team.
Comply with all state requirements and facility policies, procedures and standards of conduct.
Have the ability to assess, plan, implement and evaluate nursing care according to the individualized needs.
Guides the staff and ensures tasks are being completed.
Monitors and reports changes in residents’ symptoms and behavior.
Communicates and collaborates with the interdisciplinary team and or physicians.
Reports to the Director of Nursing

QUALIFICATIONS:

Current RN license
Reliable transportation and demonstrate excellent work ethic.
Must have excellent interpersonal, communication and organizational skills.

Job Type: Per diem

Shift: 3:00PM-11:00PM & 11:00PM-7:00AM must be available to work every other weekend

Salary: $55.00 - $60.00 per hour (commensurate with experience)

Dietary Aide (Full-time Temp)

Lynbrook Restorative Therapy and Nursing is seeking a Full-time Temporary Dietary Aide to join their growing team

Must be available for rotating weekends.

PRIMARY FUNCTIONS (Including but not limited to):

Aids in all dietary functions as instructed, in accordance with policies and procedures to ensure that quality food service is provided at all times
Prepares portions and serves appropriate and adequate amounts of food items, beverages and desserts for meal service according to infection control procedures.
Prepares and delivers nourishment to the units at appropriate times according to procedure in a safe and sanitary manner.
Sets up trays and/or place settings according to prescribed therapeutic diets using meal tickets according to procedure in a timely, safe and sanitary manner.
Assists with meal set-up at mealtime according to the diet card, considering therapeutic diets, food preferences and consistencies to meet each resident’s individual dietary needs.
Stores all food items and supplies according to the procedure including the storage of chemicals.
Stocks all needed items in appropriate quantities for proper functioning of the Dietary Department.
Prepares, washes, sanitizes and properly handles dishes and utensils and cooking utensils according to procedure in a timely, safe and sanitary manner.
Operates, cleans and sanitizes any equipment or carts utilized according to procedure in a safe and sanitary manner.
Cleans food service working area as assigned according to posted work and cleaning schedules. Maintains a neat, clean and orderly work environment including trash removal and cleaning of trash receptacles.
Understands and complies with posted work schedule.
Attends scheduled in-service education programs.
Discusses any problems, needs or concerns regarding the Dietary Department with the Food Service Director.
Assumes responsibility in supporting the Food Service Manager in promoting and maintaining positive employee relations and effective communications among facility staff.
Understands and adheres to all safety, sanitation, fire, disaster and personnel policies and procedures of the facility.
Performs other duties as assigned by the Food Service Director.

The successful Certified Nursing Assistant / CNA candidate will have the flexibility to weekends, and some holidays.

Responsibilities and Duties:

Under the supervision of the Nurse, the Certified Nursing Assistant / CNA provides assistance to the professional nursing staff in order to meet the personal needs and comfort of the resident. Duties will include bathing, oral care, incontinence care, ambulation, activities, administering nourishment, and some minor treatments as assigned.

TECHNICAL COMPETENCIES of the Certified Nursing Assistant / CNA:

General knowledge of the health care needs of geriatric residents.
General knowledge of Infection Control practices and Procedures including Protective Equipment and Universal Precautions
General knowledge of assisted feeding, nourishment, and accurate record keeping.
General knowledge of ADL’s and psychological needs of the geriatric resident.
Knowledge of taking vital signs and weights and recording of the same.
Ability to cooperate with personnel from all departments; remain levelheaded during emergencies and treat resident information as confidential materials

ESSENTIAL FUNCTIONS of the Certified Nursing Assistant / CNA:

Observes residents and reports any unusual condition, accident or incident to nurse.
Assists in any accident and incident investigations.
Participates in informal/formal resident care meetings.
Performs duties as instructed by written assignment which includes resident care to be provided i.e. activities of daily living, grooming and hygiene, transferring ambulation, ROM and proper body alignment, transporting to activities and scheduled appointment
Assists with serving meals, feeding residents and meal documentation
Maintains orderly resident rooms and assures that the call cord is within easy reach of the resident at all times.
Makes rounds on assigned residents at the beginning of each shift and at least every hour thereafter.
Care for clean and soiled linen.
Assists in the orientation of training of new C.N.A.’s on the floor as necessary.
Attends all required In-service training.
Performs other duties as assigned.

You will work under the supervision of a registered nurse or doctor. LPN professionals must be first and foremost qualified and compassionate.

The ideal candidate will be experienced in providing diverse basic care to those who need it while remaining patient and calm. It is a demanding job but also very rewarding.

Responsibilities:

Record a patient’s medical history accurately
Take and record measurements of blood pressure, temperature, heart rate etc.
Observe patients under treatment to identify progress, side-effects of medications etc.
Monitor patients’ condition including fluid intake and output and compose patient charts
Assist ailing patients in daily necessary activities such as eating, bathing etc.
Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks
Collect samples for lab testing as assigned
Provide emotional and psychological support when needed
Communicate with patients’ family or friends to provide advice, comfort and release instructions

Skills:

Proven experience as LPN or similar role
Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
Familiarity with hospital health, safety and sanitation standards and procedures
